NURSE CASE MANAGERS MAKE A DIFFERENCE - COME JOIN OUR TEAM!
We are seeking an experienced Nurse Case Manager. Must have RN Georgia License, Minimum 2 years of Case Management/Acute Care experience. Hospital based experience as an RN or case manager required. Experience in case management, utilization review, discharge planning.
Reviews demographic and clinical information of patient to determine appropriate level of care. Identify discharge needs, maintain accurate documentation. Obtains authorization from payor, secures a safe and timely discharge for patient.
Knowledge, Skills, and Abilities: Demonstrate proficiency in written and oral communications. Computer skills to include keyboard expertise, Microsoft Word and Outlook.
